Woodland Campground

The First Responders have put together a document that the members may request to provide health care providers with vital information in the event of an emergency. It is a two-page document that the members will fill out and keep in a Red envelope. The envelope should be kept in an accessible location so the EMS personnel would be able to get it. The document will contain information such as Names, Emergency contacts, Doctor’s name, allergies, medications, medical history. All information that is vital to ensure good complete medical treatment. This information will not be shared with anyone except EMS and emergency room personnel.

 

The First Responders recommend that everyone fill out the document. You can get the document by going to the office during business hours or going to the Website and fill out a request. The online request will consist of only your name, phone # and Block, and lot number. A member of the First Responders will deliver the document to you.
